Our Locations

California’s existing five PACE programs operate 18 PACE Centers in ethnically diverse, low-income communities.  Each center serves between 120 and 150 frail seniors with primary medical care, meals, transportation, prescription drugs, home care, hospital care, and when necessary, nursing home care.
 
Currently, CalPACE members serve over 2200 frail seniors, however, this number is anticipated to increase as our members continue to expand their programs in their communities.

Each PACE program serves designated zip codes in its service area. A participant must reside within one of the designated zip codes in order to be eligible for the program. For more information on the designated service areas and zip codes for PACE in your area, click on the PACE Program name below.
